Cancellation Policy
-Cancelling your appointment: Clients may cancel their appointment without charge if canceled 24 hours before scheduled appointment. If client no shows, reschedules, or cancels same day of appointment. Client will be charged 50% of the cost of the appointment.
-Cancellation of Membership during Initial Term: You may cancel your Membership at any time with a written cancellation notice 30 days prior to cancellation date.
-Memberships may be cancelled without a 30 day notice if you have one of the following conditions: 1.Your death or Permanent disability. 2. You have a Physician’s note stating that you are unable to receive massages. 3. Melted Massage, L.L.C. increases the cost of membership.
Membership place on hold: A member may place their membership on hold for 1-4 months if necessary during times of hardship.
-Cancellation of Membership during Auto Renew Term: If you have completed your contract term you will be placed on an Auto Renew “Month to Month” policy. You can cancel your membership at anytime with a 30 day written notice. We reserve the right to deny re-enrollment for an indeterminate amount of time if you have an un-satisfactory payment history or a history of no-show appointments.
If your membership account becomes delinquent and is not properly cancelled, the account will be referred to collections and you agree to pay all reasonable collections, agency fees, and legal costs incurred.
